Request Medical Records
In order to obtain copies of your medical records, please download and complete the Medical Records Release Form below. After completing the form, please email or fax the completed form to the fax number listed on the form.
We encourage patients to request medical record information at least 3 to 5 business days prior to needing them.
All requests for release of medical records to other parties must include an authorization form signed by the patient and/or legal representative.
